Make An Appointment

To schedule an appointment, please fill out the following Appointment Request Form as completely as possible. The form has two parts. Part 1 will provide us with the information we need to service your vehicle at a date and time most convenient for you. We will contact you to confirm all appointments.

The second part of the form includes a Drivability Worksheet and Noise Worksheet. If you are experiencing particular problems with your vehicle, the additional information provided on these worksheets will help us diagnose your vehicle. If you prefer, you can print the form and bring it with you to your appointment. Additional forms can be found in our Forms Library.

Your personal information will be used only by Hillmuth Automotive. We respect your privacy and will not sell the information to an outside marketing service.
